Change of Address/Data Form

This form is used to change the address on your account or to change other account information (some changes will require a new signature card). For simple address changes, please complete the Change of Data Form and submit at any one of our 8 convenient locations. (CHANGE OF ADDRESS/DATA FORM )

Lost or Stolen Credit Card

This form is used to report a lost or stolen First American Bank Credit Card. It is important to also contact Credit Cardholder Services to prevent any transactions from posting to your account between the time we receive this notification and you first notice the card is missing. Cardholder Services can be reached 24/7 by calling (800) 377-1809.

Lost or Stolen Debit Card

This form is used to report a lost or stolen First American Bank Debit Card. It is important to also contact Debit Cardholder Services to prevent any transactions from posting to your account between the time we receive this notification and you first notice the card is missing. Cardholder Services can be reached 24/7 by calling (800) 528-2273.

Unauthorized ACH Activity

This form is used to dispute Automated Debits/Credits to your First American Bank checking account. (UNAUTHORIZED ACH ACTIVITY FORM )

Stop Payment Form

This form is used to stop the payment of a check that you have written that that you do not want to be paid. Your Stop Payment request will be valid for 6 months from the date that we receive the completed form. Please print the form and fill out the required information and submit at one of our 8 convenient locations. (STOP PAYMENT FORM )
